Dear xxxx,
I am writing to let you know that I am pregnant and to notify you of when I want to start my maternity leave and begin receiving Statutory Maternity Pay.
I am currently xx weeks pregnant and I am due on the xx xxxxxx 2010. Please find enclosed my MATB1 certificate signed by my midwife. I plan on starting my maternity leave on the xx xxxxx 2010.
I was also wondering if it was possible to add two weeks of my annual leave to the start of my intended maternity leave. If my application for annual leave is successful my last working day will be the Friday xx xxxxx 2010 and my annual leave will start on the xx xxxxx 2010.
I intend to take Statutory Maternity Leave for 39 weeks. Should I wish to return to work sooner I will give you the required notice.
I would be grateful if you could confirm whether my leave starts on my intended date, my return to work date and maternity entitlements. Many thanks for all your support at this time.

tulokseen etta koska se on mun oikein, ei mun esim tarvitse sita anella "would like to"... tyyliin.
Pomo on sellainen lahopaa etta ajattelin etta viimeisen lauseen "back" tehostaisi sita etta han vastaa pikaisesti!

Mutta siis suurinpiirtein, ei kai tuosta voi loukkaantua? Etta olisi liian suorasukainen ja tyly, vai?
mutta vikassa lausessa ei tarttee tuota back- sanaa, siis riittää että look forward to hearing from you.
Ja riittää että sanot to notify you when I wish to start.. (ilman sitä ofiä)
"When I want to" on vähän liian vaativasti sanottu.
